Online complaint form

Before filing a complaint

  • Learn about the specific contexts in which the Commission can intervene by visiting our website.
  • Please read the instructions on the following panels before clicking “Start”. They contain important information that you need to know before filing a complaint.

Filling out the form

  • Complete the entire form in one session to avoid losing your data. All fields marked with an asterisk (*) are mandatory.
  • On the left side of each page, you will find useful information for filling out the form.
  • The Commission will review your form and contact you if any information is missing.

To file a complaint on behalf of someone else

You must have the person’s consent or be their legal representative. Legal representatives include parents, lawyers, agents, etc. 

You do not need the person’s consent to request intervention for a child or report exploitation. 

Submitting documents

After you complete the online form, you will have the option of attaching documents. Remember to prepare your documents in advance so they are ready when you submit your form or when we ask for them. 

Confidentiality of complaint forms  

If the Commission agrees to process your complaint, we must inform the person who the complaint is against (the “respondent”).  

  • We may send them a copy of the form you filled out.  
  • We will not send them copies of the documents you attached.  

For this reason, enter any confidential information (full names, addresses, etc.) in your attached documents only, not in the complaint form itself. 

After filing a complaint

You will receive an email confirming receipt of your complaint form.

For reports of exploitation however, the Commission will not update you on the process of your report. This is because people who report exploitation are not considered parties.
